The program, working in concert with other military and civilian agencies, is designed to provide comprehensive and coordinated assistance to family members with special needs, i.e., medical (such as diabetes, asthma, developmental delays); educational (such as learning disabilities); housing, community support, and personnel-type services. Enrollment in the EFMP is mandatory for family members with special needs. In addition, when all service members prepare for an overseas assignment, it is mandatory for his/her family members to be screened through EFMP prior to the PCS move. Overseas screenings are done by a registered nurse or LPN by appointment only at the Guthrie Ambulatory Health Care Clinic.

Appointments may be made at Clark Hall room B2-17 through the Social Services Assistant (SSA).

A copy of the DA Form 5888, AF 1466 or NAVMED 1300 need to faxed to (315)772-7995 or hand carried to the EFMP office and DA Form 4700 needs to be completed for the appointment. After faxing, call (315)772-4653.
